Young woman, obtunded and minimally responsive to sternal rub. Pinpoint pupils (2 mm bilaterally, reactive). Shallow, irregular respirations with snoring airway sounds; no gag reflex. Bradycardic. Warm dry skin, weak pulses, cap refill 2 seconds. GCS 7 (E2, V1, M4). No track marks or trauma.
EMS
EMS arrives with a 28-year-old female nursing student found unresponsive in the bathroom of an inpatient hospice facility where she works. Last seen normal about 90 minutes ago. Hospice staff found her slumped against the wall with an empty pill bottle labeled "Morphine Sulfate ER 30mg". She had snoring respirations on scene; we placed supplemental oxygen. No signs of trauma. Vitals: BP 88/54, HR 56, RR 6, sat 88% on room air, now 96% on a 15-liter non-rebreather, temp 36.4.
